Review without rebuilding the story

The sample weekly review card now includes the last recorded decision and the question still waiting for an answer. These are separate entries: an open question should not overwrite the reason for the current plan. Longer context remains available on the related record.

What to review

Check the open question before treating the card as ready for handover. If nothing needs a decision, a short readiness note can be more useful than adding a placeholder question. This example does not introduce automatic scheduling or change the way an organization agrees its own review rhythm.
